JAVA  – Phase IV  

In this phase of the project, you are to finish your report section.  You are to create the following reports:

NOTE: Change the names of the reports in your code to the names in bold below

·      Report 5 –Doctor and Patients List – Print each Doctor’s information along with the list of his patients (these are the primary doctors)

·      Report 6 –  Surgery history for ONE Patient-  Print one specific patient and the surgeries he/she had, Must also include the total payments he/she paid for his surgeries

·      Report 7 – Surgery history for ALL Patients – For all Patients, print the patient information along with the details of their surgeries, including the Doctor names.  Must also include the total payment each patient paid for their surgeries

·      Report 8- Surgery history for ALL Doctors – For all Doctors, print the doctor information along with the details of their surgeries, including the patient names.  Must also include the total payments they got for their surgeries

·      Report 9 –  Patients NOT having surgery – print all patient information if they did not have surgery, including the name of their Primary doctor

Create a word document with your name and date at the top.   Copy your code next. Copy a screen shot of each report running next in the document.
