For this assignment, complete a “mock” IRB form found on the Faulkner website at: https://www.faulkner.edu/current/academic-resources/center-for-assessment-research-effectiveness-and-enhancement/institutional-review-board/  You may need to cut and paste the link.  You can also  search “IRB” on the main page to locate it.  The link is at the bottom of the page and is labelled “RESEARCH PROTOCOL REVIEW FORM”.    You can make up the information on the form, though it is suggested that you be thinking about what you want to do for your mock proposal due near the end of the course, and use that information to complete the IRB form.  That way you will have already started on that assignment as well.  A very important thing to note is that ALL BLANKS MUST BE ANSWERED.  If something doesn’t apply, put “N/A” or “Does not apply” or “none” in the blank.  Your grade will reflect whether or not all blanks are answered, and the information is in the correct blank and makes sense (even though you are making it up).  You should be able to open the IRB form on your computer (.pdf), complete it, save it, then upload it here to submit the assignment.  It may not work on iOS devices, though some will if you have an app that will do so.   If you do not have access to a computer/PC, most public and university libraries have public access to them for you to use.
